Health history to bring to an IV assessment

A useful IV assessment starts with your health and the reason for the visit. Bring the information needed to evaluate a proposal rather than selecting a formula in advance.

Prepare one accurate list

Collect current medication names, supplement labels, allergies, and relevant prior treatment information. Include products used only occasionally. Bring existing records if requested and note details you are unsure about. You do not need to order tests yourself just because an infusion service is being considered.

Explain relevant previous experiences

Tell the clinician about prior infusions or injections, difficulties during procedures, and any reactions you have discussed with a care team. Describe what happened without deciding the cause yourself. Ask whether information from another clinician is needed before the assessment can be completed.

Use a secure channel

Ask how the practice wants records and sensitive information delivered. Avoid placing medical history in an ordinary contact email or public booking note. Hyperwell’s current preparation page does not collect health information. When clinical intake is available, the practice should explain its appropriate privacy and communication arrangements.

Make the conversation yours

Questions to take with you.

  • Which records do you need before assessment?
  • What should I clarify about my prior reactions or treatments?
  • How can I share this information securely?
