Coordinating hormone care with your primary care clinician

Ask each clinician what they are managing and how information will be shared. Two appointments do not automatically create one coordinated care plan.

Identify responsibilities early

Write down the clinicians involved and what you understand each one is addressing. Ask who orders and interprets particular tests, who prescribes, and who follows a concern that falls outside the hormone visit. This can prevent an assumption that another office has already reviewed information.

Agree on secure information sharing

Ask how summaries and relevant reports can be shared with your permission. Clarify whether you should request records or the practices will handle the request. Avoid using an ordinary website form to transfer medical documents. Keep your own record of which information was requested and whether it arrived.

Close the loop after a change

When a plan changes, ask which clinicians need to know and how the update will reach them. Confirm how duplicate testing or conflicting instructions will be addressed. Hyperwell’s coordination arrangements and clinical scope must be verified before care; the website does not imply that a referral or information-sharing relationship already exists.

Make the conversation yours

Questions to take with you.

  • Which part of my care is each practice responsible for?
  • How will summaries and results be shared?
  • Who should resolve instructions that appear to conflict?
